Community Workshop to Launch the Healthier You: Long Term Conditions Prevention Pilot (LTCPP) in Northwest London
Background
Launching across England on 1 January 2026, the LTCPP builds on the success of the NHS Diabetes Prevention Programme and supports people with hypertension to reduce their risk of developing additional long-term conditions through a nine‑month lifestyle programme.
In Northwest London, the first wave will involve 17 GP practices and will be delivered by Thrive Tribe as part of the Healthier You service.
To mark the launch, the NIHR Applied Research Collaboration Northwest London, hosted by Imperial College London and Chelsea and Westminster NHS Foundation Trust, will hold a community workshop in January to introduce the programme, connect local groups with the provider, and gather community insights to inform future research and evaluation.
Purpose
The workshops will act as both launch and engagement events, delivered in a community venue located in an area covered by the first wave of GP practices. The event will be tailored to the local population and hosted in familiar, trusted settings such as a community hall, libraries, or faith centres.
Who should attend?
Adults under 65 years old;
who live in NWL
with lived experience of high blood pressure or family history of high blood pressure
When and where?
30th January 2026 from 2pm to 5:30pm
at the White City Invention Rooms, Stadium House, 68 Wood Lane, London, W127TA
What to expect?
An introduction of the NHS Long-Term condition prevention programme.
Practical tips on managing blood pressure.
