Join the launch of the North West London Black African and Caribbean In-Reach Model: Your Community, Your Voice, Your Health (06.12)
We are delighted to invite you to join the launch of the Community North West London Black African and Caribbean In-Reach Model: Your Community, Your Voice, Your Health, taking place on Saturday 6th December 11am – 4pm at Brent Civic Centre.
This event marks the beginning of an exciting new approach by the North West London Integrated Care Board (NWL ICB) to bring health services directly into trusted community spaces - with a special focus on African and Caribbean communities in the first instance. The in-reach model aims to build trust, improve access, and co-produce culturally appropriate care through strong collaboration between local organisations, NHS partners, and Integrated Neighbourhood Teams (INTs).
The launch will include:
- A live in-reach event for NWL residents offering health checks, wellbeing activities, food, and community conversations.
- The official launch of the five in-reach hubs across five North West London boroughs.
We would love your organisation to take part by:
- Hosting a stall to showcase your approach to community in-reach.
- Networking with other community and NHS partners.
- Sharing ideas and experiences to shape the future of this model.
