London unveils vision for a neighbourhood health service to transform care across the capital
We are pleased to share with you, a significant milestone in our collective ambition to improve health and care in London - our plans for the development of a neighbourhood health service for London.
A key strength of this ambition lies in the leadership behind it. NHS England (London Region), London’s five integrated care boards, London Councils, the Greater London Authority, and the Office for Health Improvement and Disparities in London – with support from Londonwide LMCs – have all come together with a shared purpose and commitment to shape and endorse this work, in order to deliver meaningful, lasting change for Londoners.
Our vision for neighbourhood-based, integrated, person-centred care, that builds on the strengths already in place across London, is set out in the attached documents:
The Case for Change in London and The London Target Operating Model with Proposed Next Steps for Implementation of a new neighbourhood health service.
These documents are attached for your reference under embargo and will go live on the NHS England (London Region) website on Tuesday 13 May.
Built on months of engagement with clinicians, professionals, system and organisational leaders, patients, carers, voluntary organisations, and local leaders, the London model aims to improve population health outcomes, reduce inequalities, and make the best use of our collective resources.
Aligned to national policy direction, London’s model for a neighbourhood health service also responds to the capital’s unique complexity, challenges, and opportunities – including its diverse population, varied health needs, and inequalities. It is about coordinated, wraparound support, the kind of care that many assume exists until they or a loved one find that it doesn’t.
Achieving this will require not only collaboration but significant change: in how we plan, deliver, fund and evaluate care, and in how we support our workforce, communities, and services to thrive.
The London-wide vision will shape how we deliver a neighbourhood health service in north west London. Successful delivery depends on the continued partnership of our local stakeholders, from clinicians and care teams to councils, community organisations, residents and beyond.
This is a long-term journey, but the next 12 to 18 months are critical. We don’t have all the answers yet and we need to collectively shape the path ahead. Our plans set out what must happen now, what needs to be shaped at system and place level, and what will be developed jointly across partners.
