Understanding Community Heart Health Activities in London - Collecting Case Studies Survey

We are reaching out to community organisations like yours because you play a vital role in supporting the health and well-being of Londoners.   

Specifically, we want to better understand what heart health activities are already happening across London. We know there are many great examples of this work in London, but these are not always visible for all to learn from. 

What piece of work is this contributing to? This survey is part of the Community Engagement work stream of the London Million Hearts and Minds Challenge (MHMC), a five-year programme aimed at reducing heart disease and improving heart health across London. Cardiovascular disease (CVD) is a leading cause of early death and poor health in our communities, but much of it can be prevented. We believe that community organisations are at the heart of this effort, helping to reach people where they live, work, and socialise.   

Who should complete this? We are specifically interested in hearing from organisations and networks that focus on any of these heart health activities:  

  • Reducing smoking and alcohol consumption  
  • Improving healthy eating and nutrition 
  • Increasing physical activity and movement 
  • Information sharing around heart health  
  • Health checks and screenings (e.g. blood pressure checks) 
  • Mental health and wellbeing activities

When should this survey be completed by? This is a rapid survey, and we hope to receive as many contributions as possible prior to Friday, 23rd May (though late submissions may be possible). It should take approx. 5 – 20 minutes, depending upon which questions you choose to answer (none are compulsory).   

What will happen to my answers? Your responses will help us understand community heart health activities across London. We may share general findings in reports, but no personal details will be shared without permission.
